Adelaide Street Family Practice - Information for new patients wishing to join the doctors surgery

New Patient Registration

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from our reception team. If you wish to pre-register, click on the tab above. You will still need to sign these forms in person and provide identification at our practice in order to complete the registration process.

You will be registering with the practice rather than an individual GP, though do please let the receptionist know if there is a particular practitioner you would prefer to see.

Practice Boundaries

As part of the governments new 'White Paper' consultation, it is likely that old style 'practice boundaries' will be abolished in the coming months, which will mean patients will be allowed to register with whichever practice they wish, irrespective of where they live.

However, at the moment we are still stating the following boundaries to patients wishing to register or remain registered with the practice.

If you are not sure if your address is within our boundaries please ask at reception where this can be checked for you.

Pre-registration with the practice

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. Please note that you will still need to visit the practice in the next 2 weeks in order to sign the forms and complete your registration. This is a legal requirement.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
